The battle for Lou Koller‘s life has been far more harrowing than previously understood. In a raw and emotional appearance on the latest episode of “The Brooklyn Blast Furnace” podcast, Pete Koller, guitarist for New York hardcore institution Sick Of It All, shared graphic details about his brother’s ongoing fight against esophageal cancer, revealing that the vocalist’s weight plummeted to a shocking 119 pounds before doctors discovered the disease had returned.
Lou, who was declared cancer-free earlier this year only to face a recurrence four months later, is currently living in New Jersey under the 24-hour care of their brother Matt and his wife Connie.
Pete painted a stark picture of the physical toll the illness has taken on the frontman, who stands over six feet tall. After his initial battle, Lou had already dropped from his pre-cancer weight of 190 pounds down to 140. However, despite being told he was in remission, his condition continued to deteriorate rapidly.
“So then he’s cancer free, but he’s not gaining weight, he’s feeling shi**ier. And you didn’t see him online, you didn’t see him like this,” Pete said (as transcribed by Blabbermouth). “And then he went to the doctor, and the doctors were so alarmed, the way he looked… So Lou weighed 119 pounds. So, as soon as they saw him, they’re, like, ‘Yo, you gotta get tested.’ And they took them right then and there to do all these tests, and that’s when they found the three tumors.”
The recurrence involves three inoperable tumors—two pushing against the esophagus and one on the stomach sack near the intestines. Because surgery is not an option, Lou‘s survival depends entirely on chemotherapy. The positioning of the tumors made eating impossible, necessitating a feeding tube.
“Lou actually has a feeding tube because… So when the cancer came back, he had two more tumors that were pushing his esophagus folds so the food wouldn’t go in… So Matt and Connie had to change the feeding bag, like, every 12 hours or whatever the f**k, and they have to also inject water into him, ’cause it’s hard for him to drink.”
Pete noted that Lou has recently managed to ingest small amounts of soft food like cream of wheat and eggs, but hydration remains a struggle. “Someone’s gotta rough that guy up,” Pete joked grimly about getting his brother to drink more water.
So far, Lou has undergone two rounds of what Pete described as “brutal” chemotherapy, involving hours at the clinic followed by a 24-to-48-hour continuous drip via a port. While there were complications with liver enzymes spiking—requiring a reconfiguration of the chemo cocktail—there is a silver lining.
“The tumors stopped growing, which is great news. But they haven’t shrunk, so he has to keep going, and I think he has to do four to six more treatments.”
Pete‘s update aligns with a video message Lou shared just days ago, where he candidly described his prognosis as a standstill.
“I’m kind of at a stalemate where the tumors aren’t growing and they’re not spreading, but they’re not shrinking either,” Lou stated in his own update. “So the plan is to keep going with the treatments and see what happens. And after that, we’ll make a new plan.”
Despite the grim news, the hardcore community remains a pillar of support. A new fundraising event, “A Benefit For Lou Koller,” is scheduled for February 1, 2026, at the Lucky 13 Saloon in Brooklyn. Organized by Jimmy Ferrari and Howie Abrams, the show sold out its online ticket allotment in less than an hour, though 50 tickets will be available at the door on the day of the event.
